Many insurance
carriers require, you, the patient, to obtain pre-certification, authorization,
and/or referral for your visit. As a courtesy to you, we will gladly assist you
in obtaining the appropriate approvals.
If your health
insurance carrier requires a co-payment for service, that payment is required at
the time of your scheduled service. Additionally, any unpaid deductibles and/or
co-insurance for an insurance plan are also required to be paid at the time of
service. If you do not have insurance coverage, payment is due in full at the
time of service.
All credits as a result of online payments will be researched and returned to
patients within 30 days.
If you are unable
to meet your payment expectations, you must contact a Patient Representative in
advance of your appointment for prior financial arrangements.
